    If the answer is an inbox, a phone, or a glove box, nothing downstream can be current. Ask to see the queue, and ask who owns it.

    Can you reconstruct who approved what, and why, without asking anyone?

    Approval accountability is the difference between an audit response and an audit scramble. It has to be recorded as the decision happens, not after.

    How old is your job cost picture right now?

    Not how old it could be. How old it is today. If the honest answer is measured in days, decisions are being made on last week.
